Management of Hypertension in Primary Care

Abstract

|
The prevalence of hypertension in Korean adults is 26.9% in 2010. Hypertension is a major risk factor of cardiovascular diseases, congestive heart failure, chronic kidney disease, peripheral arterial disease, and retinopathy. The diagnosis of hypertension should be made cautiously because misdiagnosis is common due to white coat or masked hypertension. First step hypertension without any target organ diseases should be managed through life style modifi cation, including smoking cessation, weight control, abstinence of alcohol drinking, increasing physical activities, low salt diet, increasing
intake of vegetable and fruit, and decreasing saturated fat intake. Antihypertensive drug treatment should be individualized
according to the patients¡¯ risk factors. In general, a calcium channel blocker should be offered fi rst to people aged over 55
years old, and an angiotensin converting enzyme inhibitor or angiotensin II receptor blocker to people under 55 years old.
Primary care physicians should have some strategies to increase adherence to antihypertensive drug treatment.
